Azithromycin is a first macrolide antibiotic. It blocks protein synthesis by inhibiting the transpeptidation/translocation step of protein synthesis of bacteria and by inhibiting the assembly of the 50S ribosomal subunit.

PRESCRIBING INFORMATION

Indications

  • Treatment of infections caused by susceptible bacteria; in lower respiratory tract infections including bronchitis and pneumonia, in oral infections, in skin and soft tissue infections, in acute otitis media, and in upper respiratory tract infections including sinusitis, pharyngitis/tonsillitis.
  • Treatment of uncomplicated genital infections caused by Chlamydia trachomatis. It is also indicated for the treatment of chancroid caused by Haemophilus ducreyi, and uncomplicated genital infections caused by non-multidrug-resistant Neisseria gonorrhoeae.
  •  Used alone or in combination with rifabutin to prevent Mycobacterium avium-intracellulare complex (MAC) infection, or in combination with ethambutol to treat disseminated MAC (DMAC) in patients with advanced HIV infection.

Dosage

  • Adults
    For sexually transmitted infections caused by Chlamydia trachomatis and Haemophilus ducreyi: take a single dose of 1000 mg.
    For sexually transmitted infections caused by Neisseria gonorrhoeae: The recommended dose is 1000 mg or 2000 mg of azithromycin taken concurrently with 250 mg or 500 mg of ceftriaxone.
    For patients allergic to penicillin and/or cephalosporins: The prescriber should consult treatment guidelines.
    Prophylaxis of MAC infection in HIV-infected patients: take 1200 mg once weekly.
    Treatment of DMAC infection in patients with active HIV infection: 600 mg once daily.
    Other treatments: 500 mg/day (usually for 3 days, or 500 mg on the first day and 250 mg for the following 4 days depending on the condition).
  • Children weighing over 45 kg: Use the same dose as adults.
  • Elderly patients: Use the same dose as adults.
  • Patients with renal impairment: No dose adjustment is necessary for patients with mild to moderate renal impairment. Use caution when administering azithromycin to patients with severe renal impairment.

Usage

  • Azithromycin should be given as a single daily dose, can be taken with or without food.
  • Patients with a known hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any of the ingredients of the drug.
  • Patients with a history of obstructive jaundice/hepatic impairment associated with azithromycin use.
  • Azithromycin is well tolerated with a low incidence of adverse effects.

  • Itching, rash.
  • The use of azithromycin should be undertaken with caution in patients with significant hepatic disease.
  • Following the use of azithromycin in neonates (treatment up to 42 days of life), IHPS has been reported.
  • Azithromycin and ergot derivatives should not be co-administered.
  • Prolonged cardiac repolarization and QT interval, imparting a risk of developing cardiac arrhythmia and torsades de pointes, have been seen in treatment with azithromycin.
  • Acute exacerbations of the symptoms of myasthenia gravis have been reported in patients receiving azithromycin therapy.
  • Aztezan contains lactose. Patients with rare hereditary problems of galactose intolerance, total lactase deficiency or glucose-galactose malabsorption should not take this medicine.
  • Azithromycin should be used during pregnancy only if clearly needed.
  • A decision should be taken whether breastfeeding is discontinued or that treatment with azithromycin is discontinued/initiated or not, taking into account the benefit of breastfeeding for the child and the benefit of treatment for the woman.
  • Undesirable effects such as vertigo, convulsions, dizziness, drowsiness and syncope have been reported with azithromycin, which can affect the ability to drive and operate machinery.
